Written Book Report Form
Be sure to include on the top of your report:

- Your name
- Book title
- Author's name
- Number of pages in the book
Answer the following questions about the book you chose (either Fiction or Nonfiction):
Fiction:
- Write a detailed summary of the plot of the book. In this summary, be sure to describe major conflicts or turning points in the story.
- Characterize the major characters of your book (at least three). What was your opinion of each character, both as a character and simply as a part of the story?
- Describe the setting of your book. Does it change throughout the book? Explain. How did the author present the setting to his/her audience? Was this well presented-or in other words, could you picture the this place and time period in your mind? Explain.
- What was a theme from the book? Describe its importance. Did this theme help you understand the story any better? Explain.
- Describe at least three strengths of this book overall (reasons why you liked it) and three areas for change (things you didn't like).
- On a five star scale (1 being lowest, 5 highest), rate this book. Recommend that people read it or not based on your impressions.
Nonfiction:
- Discuss a detailed overview of the information presented in the book.
- What information did you already know? What information was new? What was the most interesting thing that you learned?
- If your book was a biography or autobiography, follow the steps above about character, setting and theme.
- If your book is a "how to" book or a strictly informational book, then be sure to be very detailed in your overview and what you learned.
- Describe at least three strengths of this book overall (reasons why you liked it) and three areas for change (things you didn't like).
- On a five star scale (1 being the lowest, 5 the highest), rate this book. Recommend that people read it or not based on your impressions.
